POSITION SUMMARY
ADULT PROTECTION: Performs various functions at a professional training level related to managing assigned cases, including: reviewing police reports and other documents to identify risk factors; conducting investigations; developing service plans and goals; interviewing clients and evaluating needs; conducting counseling and therapy; referring clients to appropriate agencies; providing crisis intervention; conducting field visits to assess needs and make recommendations; and preparing case and court reports, forms and other documents. Incumbent is required to be on-call.
CHILD WELFARE PREVENTION SERVICES COORDINATOR: Prevention Services Coordinator will be responsible for planning, implementation and coordination of various programs with the organization. This role involves working closely with families, youth, school personnel, Child Welfare staff, and other stakeholders to ensure the effective delivery of high-quality prevention services.
MINIMUM QUALIFICATIONS REQUIRED For I-III:
EDUCATION AND EXPERIENCE:
SOCIAL CASEWORKER I: Bachelor's degree from an accredited four-year college or university in one of the behavioral science fields.
SOCIAL CASEWORKER II: Bachelor's degree from an accredited four-year college or university in one of the behavioral science fields. One year of fulltime professional social casework experience.
SOCIAL CASEWORKER III: Bachelor's degree from an accredited four-year college or university in one of the behavioral science fields. Two years of fulltime professional casework experience.
And/or, 5-7 years of progressively responsible related experience, or
Any combination of education, training, and experience which provides the required knowledge, skills, and abilities to perform the essential functions of the job.
LICENSES AND CERTIFICATIONS:
· Valid Colorado driver's license
ESSENTIAL JOB FUNCTIONS: (All responsibilities may not be performed by all incumbents.)
· All work is assigned, planned, and devised to develop and teach professional techniques. All functions are either closely supervised or performed jointly with experienced staff until mastery of each individual job function is achieved.)
· The complexity of work is significantly restricted in scope. The incumbent is assigned very limited aspects of the overall cases while learning regulations, laws, and procedures and developing knowledge of casework techniques and methodology. Guidelines used are directly applicable to the assignments. Only the most routine case management decisions are made without supervisory review.
· All critical decisions are made by the supervisor. Close, detailed supervision is provided before and during the course of work. Incumbent keeps supervisor informed of the progress of each case.
· Secures client medical, physical, psychological and psychiatric records; evaluates information and makes recommendations and referrals; evaluates client capacities, makes referrals and monitors use of resources.
· Maintains records, progress notes, statistics and other correspondence related to work processes; develops and writes recommendations.
· Enters information into a computer terminal or otherwise prepares, routine and non-routine reports utilizing a variety of software; receives, sorts, and summarizes material for the preparation of reports; prepares work reports; relays and interprets administrative decisions, policies and instructions.
· Receives requests for assistance, complaints, suggestions, etc., and handles or refers requests to appropriate staff members.
· Serves as a liaison and/or member of various committees/teams and collaborates, persuades, presents reports to and negotiates/networks with others outside awn work area to coordinate efforts and maintain cooperative and efficient relations; attends and makes presentations at community forums and task force meetings.
ESSENTIAL JOB FUNCTIONS for Adult Protection Caseworker: (All responsibilities may not be performed by all incumbents.)
· Manages assigned cases, including: investigations allegations of abuse and/or neglect of at-risk adults; investigates Adult Protection referrals for possible abuse, neglect or exploitation; conducting home visits, assessing conditions and determining safety; developing service plans and goals; determining eligibility for services; making referrals to appropriate agency(s); providing crisis intervention; corresponding and collaborating with various agencies; remaining on-call, as scheduled, to respond to client needs; and preparing and submitting reports and related correspondence/documents and maintaining files.
· Based on investigations, initiates appropriate action to include elderly or disabled adult removal from hostile/unsafe environment. Petition to initiate guardianship proceedings, or other legal intervention service to alleviate situations.
· Works closely with attorneys and guardian ad litems regarding on-going court cases; testifies and makes recommendations to the court.
· Implements adult care plan utilizing client relatives, friends, and community support services to resolve abusive, neglectful, or exploitative situations. Provides advocacy for clients with no party to assist them.
ESSENTIAL JOB FUNCTIONS for Child Welfare Prevention Services Coordinator (All responsibilities may not be performed by all incumbents.)
· Organizes, facilitates, and participates in professional multi-system meetings with partner agencies for collaborative programming for the children, youth, and families.
· Provide case coordination and management for families and youth who have been referred for prevention services and supports.
· Receives, screens, and processes client referrals for prevention services. Performs interviews and conducts client needs assessments to obtain information regarding family history, current, situation, needs, goals, strengths, and readiness to make changes, refers client to appropriate community partners for families in need of services.
IMPORTANT JOB FUNCTIONS:
· Working knowledge of the dynamics of disabled adults and the elderly.
· Working knowledge of community resources and agencies that provide services to families, youth and children.
· Ability to assess situations and make prudent and appropriate decisions. Ability to apply conflict resolution and problem-solving skills.
· Maintains and upgrades professional knowledge, skills, and development by attending seminars and training programs and reading trade and professional journals and publications; makes presentations at workshops.
· Follows up on inquiries from various agencies/groups regarding department programs and services.
