POSITION SUMMARY

Provides a wide variety of public health nursing duties within the Moffat County Public Health Department, including:  performing, developing, and coordinating professional nursing and educational services, health protection, disease prevention, and health promotion for individuals, families, and community groups in a variety of public health nursing settings. 

 

MINIMUM QUALIFICATIONS REQUIRED:

EDUCATION AND EXPERIENCE:

  • Bachelors of Science Degree in Nursing from an accredited college or university required; and
  • One (1) to three (3) years of experience in a variety of healthcare settings preferred.

LICENCES AND CERTIFICATIONS:

  • Current State of Colorado R.N. License, unrestricted
  • Current CPR Certification, preferred
  • Valid Colorado Driver’s License

 

ESSENTIAL JOB FUNCTIONS: (All responsibilities may not be performed by all incumbents.)

  • Identifies health care needs through nursing assessments.  Serving patients either in a clinical setting or in patient’s home; determining/developing patient and family needs; developing health care plans; providing nursing services and treatments; referring patients with social and emotional problems to other community agencies.  Ensures that client records are kept secure and confidential, and maintained consistent with Health Department policies, procedures and HIPAA standards.
  • Helps the community health care team by coordinating assessment, planning, and providing of needed health and related services; participating in case conferences with physicians, hospital, and rehabilitative personnel and representative of other agencies.
  • Provides health information by instructing family in care and rehabilitation of patient; maintaining health and prevention of disease for family members; teaching home nursing, maternal and child care; providing instructions in other subjects related to individual and community welfare.
  • Safeguards health of children by participating in child health conferences and school health; providing group instructions for parents; conducting children’s immunization programs.
  • Arranges convalescent and rehabilitative care of sick or injured persons by cooperating with families, community agencies, and medical personnel.
  • Improves quality results by studying, evaluating, and recommending changes in processes; implementing changes.
  • Implements a variety of programs and services on both the patient and population level.
  • Assesses and evaluates the immunization status and needs of clients through the knowledge of current vaccine preventable disease immunization guidelines, vaccination schedules, state requirements, and client medical history.  Determines and administers appropriate vaccines, medications, and dosages according to assigned schedules, being aware of potential side effects, and contraindications and adheres to requirements for vaccine and medication storage and handling.  Provides educational services regarding immunizations to community residents as needed.  Completes appropriate client and clinic records, enters immunizations into CDPHE CIIS program to ensure correct vaccines are administered and documented.
  • Keeps abreast of changing trends and new developments in vaccine development, administration, and delivery.
  • Performs communicable disease case investigations and carries out interventions, participates in communicable disease outbreak response activities.
  • Enters pertinent data into various computer programs completing appropriate forms and documentation.
  • Assists Public Health Director in program administration.  May include collecting fees and completing billing when necessary.  Investigates, assesses, and participates in opportunities for seeking additional funds for Department programs.
  • Demonstrate core competencies within the Incident Command System structure as appropriate to this position’s functional role in a public health emergency response event. (Complete NIMS-700, ICS-100, and ICS-200).
  • Secures necessary consents. Perform home visits.

IMPORTANT JOB FUNCTIONS:

  • Maintains and upgrades professional knowledge, skills, and development by attending seminars and training programs and reading trade and professional journals and publications.
  • Serves as a liaison and/or member of various committees, boards, and/or teams and collaborates, persuades, presents reports to and negotiates with others outside own work area to coordinate efforts and maintain cooperative and efficient relations.
  • Interacts with a variety of high level individuals, both internally and within the community to provide information; disseminates departmental information and assists in resolving administrative issues.
  • Follows up on inquiries from various agencies, groups, media, etc., regarding department programs and services.
  • Works with diverse cultures including non-English speaking clients to overcome barriers to obtaining health care. Counsels and advises client using interpreters as necessary.
  • Disseminates a variety of information to various agencies, divisions, or departments via telephone, mail or FAX.

 

  • May serve as backup for other positions within the department.

 

  • Performs other related duties as assigned.
